Heres my two cents, and sorry if this has been addressed already,
Depending on the decay of combat focus and the ease of acquiring it, maybe at certain percentages passive skills be automatically available. Sort of like an adrenaline rush where the fighter is more aware and thus can get a small dodge/defend bonus? If automatic passive skills are no-go, maybe a command can take that place and fades once the percentage is below the threshold?
As for skills,
Mithas was thinking of a frenzy/focus-like one, and if its considered, how about replacing it with 'bloodlust'?
And I don't know how you guys feel about adding another combat opener for fighters, but I was thinking about a 'charge' command, where the fighter has a 3-4 second delay before hitting with increased damage. Might be too similar to backstab/ambush though.
